That's why your health plan includes Blue Cross Coordinated Care Advocate. It's a free program that connects you with a Blue Cross care team that's ready to help you with care decisions and nonurgent health concerns.

Whether you'd like advice about an upcoming surgery, information on a specific condition or guidance in selecting a doctor, we're ready to help.

Contact us for:

  • Advice on nonurgent health concerns, such as cold or flu symptoms, sinus pain or minor injuries
  • Care options for your specific needs, help understanding diagnoses, treatment plans and test results
  • Support with managing complex or chronic conditions and adhering to your medication plans
  • Help scheduling and finding transportation to doctor's appointments
  • Guidance to community resources and support groups

In addition, our care team may reach out to you directly with personalized recommendations based on your current health needs.

You can stay connected with your care team by phone or the BCBSM Coordinated Care app.

You can also use the app to:

  • Keep track of medications and appointments
  • Track your progress, read articles about your specific health needs, and view a list of "healthy to-dos" to complete to help improve your health
